Enhancements to the Health Trends Across Communities (HTAC) Dashboard: 2025 Data, New Conditions, and Advanced Mapping Features Now Available

The MN EHR Consortium is excited to announce its 2026 annual update to the Health Trends Across Communities (HTAC) Dashboard, introducing health condition estimates for 2025, expanded health condition reporting, and mapping upgrades designed to provide users with more detailed and actionable insights. The 2025 data represent approximately 94% of the state population.

As part of this annual update, the team has added estimates for newly reported health conditions, including type 1 diabetes and vitamin K administration, alongside refined metrics for deliveries and pregnancy-related conditions. The HTAC Dashboard now reports the prevalence of 70 conditions. These include combinations of conditions, such as the presence of both a mental health condition and a substance use disorder.

To align demographic reporting with federal standards, the Consortium has implemented updated race and ethnicity specifications. Beginning with the 2025 data, the HTAC Dashboard now includes a multiracial category and features an improved methodology for identifying Hispanic/Latino populations.

The Dashboard has also undergone significant technical upgrades to enhance usability and geographic analysis. Key improvements include:

•    A comprehensive upgrade to the mapping application for a more user-controlled experience
•    The addition of rural/urban designations and a new county-level map
•    A new Special Populations county-level map and the ability to analyze communities by primary
     language

These enhancements reflect the MN EHR Consortium’s ongoing commitment to push the boundaries of what is possible through its unique federated data model and to provide researchers, policymakers, and the public with precise tools to assess community health needs.

Special thanks to MN EHR Consortium members, health systems and research institutes, HTAC User Group (HUG), and state and local public health partners who have provided valuable content- and usability-related feedback.
